Revive Health Senior Care Management is dedicated to excellence, integrity and ethical behavior as it provides health care services to its residents.

If you feel that we are not doing our best to fulfill our mission, please let us know so that we can rectify the situation. We encourage anyone who has a relationship with Revive Health Senior Care Management to contact us with any feedback, including any concerns or complaints about resident care, employee relations, policy compliance, business ethics or any of the following:

  • Billing or reimbursement issues

  • Fraud/theft

  • Violation of governmental regulations

  • Workplace violence

  • Discrimination

  • Harassment

  • Conflict of interest

  • Substance abuse

The first step in resolving concerns or disagreements is to approach the community, team or individual directly, and our leadership at a local community is available to assist.
